Requirements for MMJ Card in WA as per RCW 69.51 A (1+ of the following):
*Pain, severe, or intractable
*Chron’s
* Hepatitis C with nausea or pain
*Severe nausea
*Appetite loss
*Abdominal cramping
*Anorexia
*Muscle spasms
*HIV
*Epilepsy
*MS
*Seizures
*Cancer
*Glaucoma
medical records are required to verify such condition(s) is/are chronic. therein lies the problem, for me.
1. i DO have chronic and severe back/neck pain, i DO have muscle spasms; they are for the most part restricted to the muscles in my forearms and cause "shaky hands" however they also cause minor issues with my ability to talk and sing, i DO have severe chronic insomnia, and i HAVE had all of these conditions since i was a young child (primary school/elementary). what i do not have are the medical records to prove it, nor have i tried any medical treatment for any kind of extended period of time. the reason? pretty simple, i hate doctors. i realize that they are vital, i know and respect their job to a degree very few others outside the medical field can compete with (i am very much interested in bioengineering as a career and have a fondness of anatomy and biology). but they are simply not for me.
2. i can prove, given the time required, that these conditions are very much real and chronic. when i was in high school i self medicated with marijuana, i wont lie, i had fun too, but for the most part i smoked before i went to bed so that… well, so that i could actually go to bed. there was also a very drastic change in my ability to speak and write/draw, my handwriting is much like that of the average doctor when sober. obviously the pain in my spine subsided.
3. throughout the years there are times in my life when i have gone and taken prescribed medications due to these conditions becoming temporarily far more severe. i have been prescribed muscle relaxants usually reserved for people with scoliosis, as well as oxycodean and vicodin, for the pain in my back/neck. as for sleep, i have never been prescribed anything for that however throughout high school i had an average of three hours of sleep a night.
4. now let me cut to the chase. i do not have records proving these conditions i have are chronic. i also do not have records proving i have tried other medications to treat them. i do not have these records because anything i was prescribed was on a temporary basis. what do i need to do to get these records?

You definitely have to have medical records to prove a medical condition. In Colorado you do.

You don’t need any records at all.
Talk to your doctor about why mmj would help your problems and ask him to sign your
authorization form (which you can Google and download online.)
If he won’t sign it for you, you’ll need to find one that will,
by either changing doctor
or going to a mmj doctor for a one time visit to get your paperwork signed.
It will cost you 99-200$US.
